When children and families enter the early intervention program, an important outcome is to prepare for the transition to the next program. The process of transition planning and preparation starts long before the formal transition process begins. As we begin to develop relationships with parents, we can begin to help them think about transition and what that might look like for their child.
What we’d like to know:
• When do you start the conversation about transition with parents?
• How do you make sure that families have the information and resources necessary to be comfortable making decisions for their child?
• What are some ways that you help families feel more confident about moving out of early intervention?
• What are some mistakes you’ve learned from?
• What are some “bright spots” you’ve experienced with families

The New Mexico Early Childhood Transition Initiative, jointly funded by the NM Dept of Health FIT program and the PED Special Education Bureau, supports community teams in developing and/or improving their system of transitioning children and families from early intervention services to other services and supports.
